Light for Life
Light for Life provides a range of services to support people in Sefton. As part of SSHG, Light for Life is based at the Hub@Southport with staff also working at various other locations across the borough providing services to support the most vulnerable people in the community. The Hub@Southport provides a friendly and welcoming environment, where people can access a range of support.
The Rough Sleeper Service engages those who are sleeping rough, providing routes into accommodation appropriate to their needs. Rough Sleeper Community Services operate across different locations in Sefton, with provision for food, showers and laundry facilities, and access to health services.
The Navigator Service provides intensive support to those with multiple complex issues living in SSHG accommodation. The Hospital In-Reach Service provides face-to-face assessments with patients for accommodation with full health-screening and vaccination programmes, and links into social care, housing and probation, to ensure that patients have safe places to go upon discharge.
The Women’s Hub is operated in partnership with SWACA and SWAN, providing a range of services for women experiencing domestic abuse and other challenges. The Women’s Hub is based at a central but discreet location in Southport accessible by public transport from all locations across north Sefton. The Health and Wellbeing Service is facilitated in partnership with Southport and Ormskirk NHS Trust, Merseycare, CGL and St Mark’s Medical Centre.
